[Medical student,Internist] Pyramidal weakness (UMN) ผู้ป่วยชายอายุประมาณ 60 ปี มา Admit รพ. ด้วย ขาด้านขวาอ่อนแรง เป็นมา 3 วัน ก่อนหน้ามา รพ. ประมาณ 7 วันก่อนตรวจ จริงๆผู้ป่วยรายนี้มีอาการชาขึ้นมาจนถึงบริเวณหน้าท้อง (Sensory level) ประมาณ T10 ซึ่งทำให้สงสัย cord lesion อยู่แล้ว แต่ถ้าจะประเมินเรื่องขาอ่อนแรงอย่างเดียว ก็พอบอกได้ว่าเป็นอะไร Monoparesis of leg DDx - Upper motor neuron เช่น Contralateral parasaggital lesion (Frontal lobe) ซึ่งอาจทำให้ขาอ่อนแรงได้แต่จะไม่มีอาการชา เนื่องจากรอยโรคคง
